U.S passport must be valid at least 6 months beyond intended stay and must have at least one blank visa page.  If your passport is not valid 6 months beyond your intended stay, you must renew your passport and then we will secure your visa with the new passport.

NOTE: Passport cannot contain an Israeli stamp

Please send 2 recent professional passport-type photographs.
Digital and non-passport photographs are not acceptable.

Proof of US Status

Photocopy of Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport)

Minors

A business letter of responsibility (sample) is required, to be typewritten on your company letterhead and addressed to:
Consulate of New York, Visa Section, New York, N.Y
The letter must explain all of the following:
Nature of business to be performed
Name and address of reference to be visited
This letter should request Multiple Entries visa if required
Guarantee of return transportation and sufficient funds for the visit
The letter should be signed by an officer of the company other than the applicant.

Applicants of Lebanese origin must present copy of Lebanese ID card.

Covering letter specifying the length of stay and purpose of the trip.(Required for Tourist Travel)

Spouse of applicants of Lebanese origin must submit a copy of the marriage certificate and a copy of the spouse's Lebanese ID.
