Please send TDS your current valid and signed passport. It must not expire within 3 months and must have at least one blank visa page.  If your passport expires within 3 months, you must renew your passport and then we will secure your visa with the new passport.

Please send 2 recent professional passport-type photographs.
Digital and non-passport photographs are not acceptable.

Photocopy of Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport)

Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant's name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

Invitation letter from Host Company or Organization in Azerbaijan, authorized by the Azerbaijan Ministry of Foreign Affairs. If an invitation is not sent through Consular Department of the MFA, travelers should submit an invitation received by fax directly from a host company or organization in Azerbaijan and a letter of request from the employer

For tourist visas (only single entry): confirmation from receiving tourist organization in Azerbaijan or from a hotel is required. Tourist visa is valid for exact dates of hotel accommodations purchased in advance.

For visitor visas (only single entry): if invitation from the citizen of Azerbaijan is not sent through Consular Department of the MFA, the applicant has to state exact address and phone number of inviting party in the application form and to enclose it a copy of Azerbaijan citizen`s identity document.

NOTE: The Embassy of Azerbaijan is closed on Tuesdays Wednesdays and Fridays. Submission and collection of applications can only occur on Mondays and Thursdays.
